Overview: Dental implants have been successfully implemented as a routine therapeutic approach in partially and fully edentuous patients. Alveolar defects compromise the “prosthetically-driven” dental implant positioning, unless a detailed planning is performed prior to the surgical phase. Many techniques have been described to augment alveolar bone, including autogenous block grafts, titanium mesh, distraction osteogenesis, guided bone regeneration, just to name a few. The use of Guided Bone Regeneration associated to non-autogenous scaffolds constitutes the standard of care of bone augmentation in all alveolar defects with or without implants, including the most severe vertical deficiencies. Choosing the correct therapeutic approach with the appropriate biomaterials is a fundamental player to achieve success in advanced GBR procedures.

Overview: True therapeutic success is more than fabrication of a highly aesthetic crown, periodontal pocket elimination, or adequate bone regeneration for implant placement. Maximation of patient outcomes mandates insightful diagnosis, thorough case work-up, development of a comprehensive interdisciplinary treatment plan, exquisite execution of therapy, and appropriate maintenance care. It is imperative that e employ unforginving definitions of success, both in each component of therapy and overall treatment results. This presentation will demonstrate the effectiveness of periodontal pocket elimination therapy through documented results of over 30,000 teeth treated and followed for 15-35 years. Succesfful treatment outcomes following bone regenerative and/or implant therapies will be discussed through the examination of over 4,000 regenerative cases and 25,000 implants which have been placed and restored. Decision trees will be formulated for when to perform pocket elimination therapy, whether it be through resection, regeneration, or a combination of both; and when to extract teeth with or without immediate implant placement in anterior and posterior areas. All treatment outcomes will be discussed within a framework of how to best ensure patient acceptance of comprehensive care. The future is now. We have the knowledge, techniques, and technologies to predictably bring treatment and therapeutic otcomes to new heights. The greatest challenge is our understanding.
