Suddenly
we find ourselves in a new paradigm of dentistry: medical management of caries.
Non-operative approaches have resurged around Silver Diamine Fluoride (SDF),
which enables the treatment of cavities by Dental Hygienists. Clinical trials
and safety studies have been completed, and FDA clearance of SDF in 2014 has
spawned support from all major dental groups. These changes have brought about
new opportunities and new challenges. While SDF is fast and non-invasive,
monitoring outcomes is fundamentally different than for restorations. Clinicians need to be prepared to
approach the black staining, the 11-32% of carious lesions that keep growing,
and to prepare patients to receive these treatments. Lessons from 4
years of use in the US can guide clinicians to dramatically reduce the burden
of caries in any population.
Combination
with glass ionomer cement (GIC) addresses most of the shortcomings of SDF. GIC
has been shown to be equivalent to resin for dental sealants, and is the basis
of ITR/ART fillings and the Hall crown technique. These procedures are each increasingly
performed by Hygienists.
Regarding
brush-on preventives, much has been learned in the past decade. Does Fluoride
varnish actually work in young children? What about Xylitol? Surprising news
will be unveiled.
In
summary, we will cover the evidence and clinical use for SDF and GIC in caries
treatment, and dive into the options to prevent cavities from forming in the
first place. We will address
clinical trial data and mechanistic studies, rank the effectiveness of the options,
and elaborate clinical protocols with hands on experiences.

At the conclusion of the session, the
participant will be able to:
1. Explain the indications, benefits, and
risks for SDF, ITR/ART, SMART, and Hall crowns to patients.
2.
Describe the scientific basis for SDF in treatment or prevention of dental
caries.
3.
Demonstrate the procedure for applying SDF to cavities.
4. Demonstrate the technique for
placing dental sealants with high viscosity glass ionomer cement (GIC).
5. Select effective non-invasive treatment and prevention strategies
for your communities.
6.
Convince dentists that Hygienists are now an essential part of caries
management.
